HCS Serious Incident Reports
The reports have been redacted to ensure that no personal information is released in accordance with Article 25 of the Freedom of Information (Jersey) Law 2011.

Quarterly reports include major accidents, minor incidents and minor accidents. Please see the attached data obtained from the corporate reporting incidents and accidents within CYPES and IHE. Major incidents within IHE have been classified using the Health and Safety Executive (HSE) Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR) 2013.
CYPES and IHE reports
Disclosure control has been applied to major accidents, minor incidents and accidents with less than five cases as it would likely breach the privacy of individuals. Article 25 (Personal Information) of the Freedom of Information (Jersey) Law 2011 has therefore been applied.
Please note that reporting procedures were updated in October 2020. The data for this period until June 2021 is represented in a separate table to reflect this.
The figures in the tables from October 2020 have had a significant increase due to the way the reporting has changed. This is more noticeable within CYPES as the severity of the threshold decreased to enable more data to be captured.
The new reporting was also rolled out to all nursery, primary and secondary schools. These incidents would have previously been recorded locally. CYPES also took on the remit for Children’s Services, which is now also using the new reporting system. Again, these figures were previously recorded separately.
Please also note that prior to Q4 2020, incidents were only reported to the Chief Operating Office on employees. Now, all incidents are recorded for employees, visitors, services users, students and so on.
Reports for the period up to October 2020 show all reportable incidents, regardless of whether an ambulance was called or attendance to Accident and Emergency (A&E) was required.

(1) Information is absolutely exempt information if it constitutes personal data of which the applicant is the data subject as defined in the Data Protection (Jersey) Law 2018.
(2) Information is absolutely exempt information if –
(a) it constitutes personal data of which the applicant is not the data subject as defined in the Data Protection (Jersey) Law 2018; and
(b) its supply to a member of the public would contravene any of the data protection principles, as defined in that Law.
