Details of any discount, rebate or reimbursement your organisation receives in respect of Blood Glucose Testing Strips

B

A copy of and or a link to your current blood glucose testing formulary / recommended blood glucose meters / strips

C

The planned date to next review your blood glucose meters / strips formulary

D

The name(s), contact email address(es) and telephone numbers of the person(s) in your organisation who are responsible for blood glucose meter / strip formulary / recommendations within your organisation

E

How will the formulary review be managed, what will be included in the process and anticipated timelines for completion?

F

Are you working with any other Critical Commissioning Group (CCG) / Primary Care Organisation (PCO) who will be part of this process or who will adopt the same Formulary?

If so, please provide the names of these areas and the contact name, email address and telephone number for the person with responsibility for blood glucose meters.

A

Blood glucose testing strips are provided to eligible patients under a contract with community pharmacy who are reimbursed based on prices contained in the UK Drug Tariff with no discount applied.

B

Blood glucose strips and diabetes ancillaries are not included on the Jersey Prescribed List (formulary). Supplies in receipt of government funding are included on a ‘catalogue of items’ and are provided via community pharmacy under contract. These may be viewed on the link below

There are no plans to place meters or strips on the formulary. The next planned review of the Diabetes Ancillaries Service is November 2021.

Queries and recommendations regarding the diabetes ancillaries service may be directed to h.diabetesnurse@gov.je

There are no plans to place meters or strips on the formulary. The Service will be reviewed by gathering feedback from service users, practitioners, and specialist advice alongside management information regarding volumes and cost.

The service does not partner with other Critical Commissioning Group / Primary Care Organisation.
