Islanders who are living with COPD, Heart Disease, Diabetes, or asthma can now access the my mhealth app by using an online self-referral form.
The my mhealth app, which connects patients with their clinical teams and enables healthcare professionals to offer more accessible and efficient services, was previously only available to high-risk individuals, but is now open to all who are living with the above conditions.
The app helps Islanders track, manage, and understand their conditions and gives easy access to self-management and rehabilitation tools.
Islanders who are living with these conditions can complete the online referral form and request access to the app here.
Once completed, Islanders will receive an activation email from my mhealth with step-by-step instructions on how to set up the app and make it work for them.
Digital Nurse and Project Lead, Ricardo Da Silva, said: "This is a great step in ensuring Islanders can access the support they need at any time. We have received extremely positive feedback from patients which has led to us opening it out to more Islanders.
The app allows patients to take control of their experience and to share data with their clinicians to help them manage their treatment together. We are excited to be extending this to all Islanders who are living with COPD, asthma, diabetes and heart disease."
