Households in Jersey are being selected at random to take part in a one-off Health, Activity and Wellbeing Survey run by the Public Health Department.
The survey should be completed by whichever person in the household is aged over 16 and has their birthday next. Questions will cover health, diet, lifestyle, physical activity, wellbeing and volunteering, and aim to capture Islanders' experiences during the COVID-19 pandemic.
Director of Public Health, Professor Peter Bradley, said: "The selection of households aims to capture a representative group of Islanders based on our population. We want to use their direct experiences to help shape future public health policies and inform investment in sports and exercise facilities.
"It is important we understand how COVID has affected people's health and wellbeing so we can work on recovery policies to help Islanders sustain healthy lives."
This year's survey has been developed in partnership with Jersey Sport, who lead on the development and implementation of the Inspiring an Active Jersey Strategy. All survey responses will be combined to produce an anonymous report which will be published before the end of the year.
Chief Executive of Jersey Sport, Catriona McAlister, said: "The pandemic has affected us all in one way or another and this survey aims to gather an evidenced understanding of that. Many activities were temporarily paused but many people also took up new activities. The information provided will be invaluable in helping us to understand the impact of COVID-19 on Islanders' mental and physical health, and in shaping our response.
"If your household receives a letter this month, we ask that the person in your household, whose birthday is next, completes it online by using the unique reference code provided."
Letters will be sent to households this week.
Completed surveys will be entered into a prize draw to win £250.
