How many internal disciplinary investigations were carried out by the States in the years 2012, 2013, 2014 and 2015?

How many have been carried out to date in 2016. And how many are ongoing?

What action has been taken as a result, in general terms? For example, how many staff were given written warnings, how many dismissed, how many had no action taken etc as a result of those investigations? Please break all figures, where possible, down into departments.
