Numbers and reasons for senior staff leaving (FOI)

Could you please tell me how many civil servants of pay grades left their positions by year from 2012 to 2017?  

• grades 12/0 to 12/3

• grades 13/0 to 13/3

• grades 14/0 to 14/3

• grades 15/0 to 15/3

Could you also tell me which positions they vacated, and why they left it?

I am aware that you are not able to identify individuals, so please give me as much information as possible without doing so.
